Our focus is on community health outcomes — specifically, greater opportunities for people to achieve their fullest health potential, leading to health equity across the state.
We believe in changing the system and responding to emerging needs. Through a careful planning process that includes input from grantees, the broader community and our staff and board, we identify specific focus areas where we determine that our grantmaking can make the greatest impact. While we fund only in specific focus areas announced through our Request for Proposal process, the backdrop to our work is the research showing that the social determinants of health drive health outcomes:
